PARENT / GUARDIAN CONSENT
By registering my girl for A Girl’s H.E.A.R.T., I acknowledge that:
♥ I am the parent or legal guardian of the participant.
♥ I have provided accurate and complete information.
♥ I authorize program staff to obtain emergency medical care for my girl if I cannot be reached.
♥ I understand it is my responsibility to keep all information up to date.
♥ I agree to the program policies, code of conduct, and safety standards.
♥ My consent allows my girl to fully participate in all program activities.
This consent remains in effect for the duration of her participation unless I notify A Girl’s H.E.A.R.T. in writing.
